Whether your preferences have changed, your lifestyle is different, or your breast implants no longer feel right for you, you deserve a safe and supportive experience. At San Antonio Plastic Surgery Institute, Dr. Michael E. Decherd and Dr. Matthew Isakson guide you through every step of breast implant removal with compassion, precision, and a focus on achieving natural, balanced results that reflect who you are today.

Why You Might Choose Breast Implant Removal

Every woman’s reasons are unique, but some of the most common include:

  • You prefer a smaller or more natural breast size
  • Your breasts have changed after pregnancy, weight fluctuation, or aging
  • Your implants have ruptured, shifted, or developed capsular contracture
  • You feel discomfort, tightness, or asymmetry
  • You want to replace your implants with new ones (Breast Implant Revision)
  • You have concerns about symptoms related to Breast Implant Illness (BII) and would like to discuss your options in a safe, informed setting

For patients with saline implants, your surgeon may recommend deflating the implants prior to removal. This allows you to see approximately how your breasts may look without implants and can help you decide whether to proceed with simple removal, a lift, or replacement.

During your consultation, you’ll discuss your goals and concerns directly with your surgeon so your plan fits your body and your vision.

Breast Implant Removal Techniques

At San Antonio Plastic Surgery Institute, Dr. Decherd and Dr. Isakson perform implant removal with utmost safety and individualized planning. The technique chosen depends on your anatomy, the condition of your breast implants, and your desired outcome.

Simple Implant Removal

The implants are removed without disturbing the capsule (the natural scar tissue surrounding them). This approach may be recommended when the capsule is soft and healthy.

If you have saline implants, your surgeon may recommend a deflation procedure prior to removal. Deflating the implants in advance allows you to see approximately how your breasts may look without implants and can help you decide whether to proceed with simple removal, a lift, or replacement.

Capsulectomy

Both the implant and portions of the capsule are removed to improve breast comfort, shape, or symmetry. This is often performed when scar tissue has thickened or hardened.

Total Periprosthetic Capsulectomy

The term “en bloc removal” is commonly used online, particularly within the Breast Implant Illness community. However, in most cases, your surgeon will perform what is called a total periprosthetic capsulectomy, removing the implant and all or most of the surrounding capsule tissue in one piece when appropriate. A partial capsulectomy may also be applicable in select cases, and your surgeon will review this option with you during your consultation.

What Is Capsular Contracture?

Capsular contracture occurs when the natural layer of scar tissue (the capsule) that forms around a breast implant becomes unusually tight or thick. This tightening can cause the breast to feel firm, appear misshapen, or even cause discomfort. In these cases, removing the capsule and implant together can help restore softness, symmetry, and comfort.

Breast Lift (Mastopexy)

If your breasts appear lower or deflated after removal, your surgeon may recommend a Breast Lift during the same surgery. A lift restores shape and firmness for a more youthful contour.

What to Expect During Surgery

Your breast implant removal is performed under anesthesia in our private, on-site surgical suite. Most procedures take 1.5 to 3 hours, depending on the complexity of your case and whether you combine removal with a lift or revision.

You’ll have the same experienced team by your side from start to finish, ensuring a smooth and comfortable experience. Before surgery, your surgeon will review every step with you so you feel informed and confident.

Considering a Breast Lift After Removal

When implants are removed, your skin and breast tissue may not fully retract on their own. A breast lift can:

  • Lift and reshape your breasts for a firmer, balanced look
  • Remove stretched or excess skin
  • Reposition your nipples and areolas for a natural proportion

You and your surgeon will decide whether to combine a lift with removal in one procedure or schedule it separately based on your anatomy and recovery goals.

Recovery and Results

Most patients return to regular activities within 7 to 10 days. If your job involves physical activity, you may need two to three weeks before resuming heavier tasks or exercise. Full recovery and final settling typically occur within six to twelve weeks.

During the first few weeks, you can expect mild swelling, tightness, and changes in sensation. These effects gradually fade as your body heals. Your results will continue to refine over time, revealing a natural, balanced appearance that fits your current stage of life.

Our surgeons and staff follow you closely after surgery, with scheduled check-ins at one week, one month, and three months post-op. You’ll have direct access to your care team at every stage.
